• Licensed professional Social Workers will coordinate an evaluation and follow-up visit(s) as necessary to provide patient/caregiver with resources available in the community and overall emotional and social support to assist with the issues related to the patient/caregiver’s health problems
• Coordinate and referral to medical and non-medical services available in the community to assist with the patient’s condition (ie. special transportation, home delivered meals, application to supplemental community programs for increased assistance as needed, etc.)
• Educating patient/caregiver on available and alternative options for another level of care as necessary with relevance to patient/caregiver’s condition.
