Mental Health Home Services are delivered to customers in their homes. (The residence has no hospital, skilled nursing facility, day rehabilitation or residential rehab facility, or adult day care facility.) This care is provided by a licensed registered nurse to customers who are unable to access office-based services. This is necessary to continue and maintain a comprehensive plan of care.
This service is only available for home-bound. (“Home-bound” is defined as a person whose medical or psychiatric condition limits their ability to safely leave home without the assistance or supervision of another individual or without the help of a supporting device and the patient’s home only leave for medical / psychiatric treatment or receive home leave often for non-medical purposes. “)
Typically, the patient has a very poor support system within the community (no family or interested party to act as concierge, family members or interested parties exist, but have poor understanding of psychiatric disorder of the client and to help have no positive effect on the improvement of the psychiatric / medical condition of the client). Services may include drugs administration, assistance in setting up a medical system, education and the monitoring of medication, and observation of the physical well-being in relation to medication side effects.
The service is designed to facilitate support and increased coordination with rehabilitation services, such as community support services. This service is not intended the direct involvement of a physician / psychiatrist to be replaced in the treatment of the individual.
