Medical Case Management

Medical case management is a process that treatment and rehabilitation plans for people facilitated with identified needs (disabled, injured or ill). It is a collaborative process and involves health care evaluation, assists in the implementation and coordination of the subsequent care plans, regular monitoring of progress and the evaluation and assessment of results. The goal is to provide patients to improve as much as possible, their independence and quality of life.

Senior Care Management

Each patient will be unique and therefore each situation and treatment plan will be different. Some people will need case management services for life, while others have only needed for the medium term. Often the customer has a change in circumstances in which they need help to deal with, like a brain injury. Sometimes they are dealing with health problems from birth such as cerebral palsy. An individual will be responsible for the overall management of a patient and this person will be allocated on the basis of suitability.

Case managers come from professional backgrounds such as nursing, social work, occupational therapy, and physiotherapy.

The first part of their work is the judgment. Relevant information must be collected from the patient, family members and medical practitioners. The purpose of the assessment is to develop an appreciation of the individual requirements and to plan the how to reduce the impact of their situation on their degree of independence, personal safety, finances, family life, social life and the risk of a reduce job.

The task of case management can involve many facets including advocacy, assisting families with securing funding to help them communicate with external entities, attending meetings with them and making families aware of all relevant support groups. The task also includes making sure that everyone understands the medical and legal jargon utilized.

Rehabilitation goals are identified and the most appropriate and cost-effective care is planned. The case manager will create and track the progress and reviews of care takes place at times determined by the customer or acting agreed for them.
